To schedule any follow up appointment, click here to access your Charm Portal.

Login to your portal and click on the ‘Appointments’ button.

Next click on the ‘+’ sign in the upper right corner, this will allow you to schedule your follow up appointment.

Please note that if it has been more than a year since your last appointment, a 60-minute follow-up is required to make sure we have enough time to catch up on all of your history and symptoms.
